If paying by check or money order, please print and complete the form and mail it with your payment to be received no later than July 18, 2012. Early Bird Special by July 5, 2012. Only paid registrations are guaranteed enrollment in the workshop. No refunds after the first workshop session but fees can be applied to a future workshop or transferred to another participant. PayPal, credit card and ATM payments are registered the same day and this enrollment form is not necessary.
